Serving more than 1,200 Neighbors annually across North Phoenix, Carefree, Cave Creek, and surrounding communities, FCC provides practical, compassionate support that reduces isolation, addresses barriers to daily living, and strengthens overall well-being.
FCC offers a continuum of services designed to meet the evolving needs of Neighbors as they age or navigate disability. Programs include transportation to medical appointments and essential errands, caregiver support, mobile meals, friendly visiting and reassurance calls, wellness and social engagement activities, information and referral services, and volunteer-based assistance. By connecting Neighbors with trusted volunteers, community resources, and personalized support, FCC helps individuals maintain their independence while remaining active and engaged in their communities.
Many of the Neighbors served are older adults living alone, individuals managing chronic health conditions, or family caregivers balancing significant responsibilities. FCC's services help prevent social isolation, improve access to healthcare and nutritious food, reduce caregiver stress, and enable Neighbors to age in place safely. Through a compassionate, person-centered approach, FCC empowers Neighbors to remain connected, healthy, and independent, improving quality of life while strengthening the communities they call home.
