Guidelines:

• Review the five alumni award descriptions and criteria before selecting the award for which you will submit a nomination.
• Create a login (email & password) to submit your nomination.
• You may save your work and return later to complete your submission before the Wednesday, April 30, 2025 deadline.
• Provide information for each criteria. Enter "NA" if your nominee does not meet a criteria. What you enter for the criteria is the most important part in the evaulation and selection process.
• You may add up to 10 attachments or URLs to support your nomination, but please do not rely on these alone.
• You may make multiple nominations. A new submission form is required for each. Self-nominations are permitted.
