The following items must be submitted in order for your nomination packet to be
considered complete:
● Completed nomination form
● A copy of the nominee’s resume, CV, LinkedIn profile, or similar professional
summary
● 1-2 letters of recommendation (from individuals other than the nominator)
● NOMINATION DEADLINE: SUNDAY, JANUARY 8, 2023
